Prevention Infrastructure Worksheets - Optional worksheets for mapping existing  alcohol, cocaine, and methamphetamine programs/services to determine the risk/protective factors and intervening variables already being addressed.  This tool may be helpful in identifying gaps in services.

Prevention Resource Assessment Detail Sheet- An optional tool for recording detailed information of existing community programs/services.  This tool may be helpful in completing the Prevention Infrastructure Worksheets.

Epidemiological Profile

Epi Profile Worksheets - An optional worksheet for mapping epidemiological data onto the logic model for alcohol, cocaine, and methamphetamine use.  This tool may be helpful in writing the epi profile.

Risk & Protective Factors – Example of operationalized indicators of risk & protective factors.

National Outcome Measures (NOMs) Survey – Optional survey that may be collected at the community level during assessment phase to serve as a baseline for monitoring drug use trends over time, comparison with state telephone survey conducted by state evaluators or comparison with local program evaluation results.  The survey is available in WORD format (English / Spanish) or Survey Monkey (English / Spanish).

Alcohol and tobacco-involved births. Self-reported tobacco smoking and use of alcohol during pregnancy by new mothers, county and state-level counts and rates, from birth certificate data collected and reported by ISDH (Epidemiology Resource Center Data Analysis Team).
