National Outcome Measures (NOMs) Survey
Optional survey that may be collected at the community level during assessment phase to serve as a baseline for monitoring drug use trends over time, comparison with state telephone survey conducted by state evaluators or comparison with local program evaluation results. The survey is available in WORD format (English / Spanish) or Survey Monkey (English / Spanish).
